Be sure to bring this history form with you to your first visit. This will cut down on your office time and allow you to move along toward diagnosis and treatment as soon as possible.

The American Board of Phlebology, following its third successful examination and the approval of 477 certified Diplomates since the program began, looks forward to congratulating the fourth class of certified phlebologists in the summer of 2011. The establishment of a Board Certification Exam brings recognition to both the field of phlebology and those providers in the field who have the knowledge, skills, and experience to provide quality care to phlebology patients.

The primary purpose and most essential function of the Board is to protect the public by establishing and maintaining high standards in thoracic surgery. To achieve these objectives, the Board has established qualifications for examination and procedures for certification and Maintenance of Certification. The requirements and procedures are reviewed regularly and modified as necessary
Mesa View Regional Hospital is a 25 bed Critical Access Hospital located in Mesquite Nevada.
The hospital opened its doors on July 13, 2004 and provides a full-range of inpatient and outpatient hospital services including an Intensive Care Unit, Obstetrics, Medical, Surgical, Laboratory, Radiology and Diagnostic Therapy Services. ER services including trauma are provided 24 hours seven days every week
